This clutch was removed from a Volvo V70 that was recovered in because the customer lost all drive and gear selection.
The picture shows the old unit compared to the new one, as you can see the pressure plate has cracked caused by excessive heat, The friction plate has also been subjected to excessive heat, causing the friction material to disintegrate.
Clutches are classed as friction part and over time, and mileage, they will wear out, however this particular case is quite extreme. Clutches can also wear due to the driver resting their foot on the clutch pedal.
Typical signs of a clutch becoming worn are, the clutch "biting point" becoming high, the pedal stiff in operation or the gear selection becoming difficult. Anotehr obvious sign is that the engine starts to rev but the car does not move.
